Association Celebrates ‘Case Management: We Listen. We Care. We Lead.’ During National Case Management Week, Oct. 9-15
Allsup joins American Case Management Association in recognizing health system case managers’ achievements
Belleville, Illinois — Oct. 4, 2016 — Professional case managers are the catalysts who successfully support patients across the continuum of care offered by today’s health care delivery systems, according to the American Case Management Association (ACMA). ACMA and Allsup will recognize health care case managers during National Case Management Week, Oct. 9-15. Allsup is a nationwide provider of Social Security Disability Insurance (SSDI) representation, veterans disability appeal, exchange plan and Medicare plan selection services.
ACMA’s 2016 theme is “Case Management: We Listen. We Care. We Lead.” and emphasizes the variety of responsibilities required of hospital case managers and transitions of care professionals. These include clinical assessment, care coordination, patient education, counseling, clinical pathway management, discharge planning, resource management and patient advocacy.
National Case Management Week provides professionals, health care organizations and communities across the U.S. with the opportunity to recognize, support and encourage the ongoing achievements of case managers providing excellent service to patients and families.
The American Case Management Association offers solutions to support the collaborative practice of hospital and health system case management. The 2016 ACMA Leadership Conference will be held Thursday and Friday, Oct. 27-28, in San Diego.
